I-693

Change of status

The United States Citizenship and Immigration Services (USCIS) requires an I-693 to be completed for anyone seeking to apply for Permanent Residency or change of status be completed by a Civil Surgeon. The medical examination is to confirm a person(s) health status. Requiring that all physical and mental disorders are addressed, and all the necessary required vaccinations have been obtained.

The immigrations requires that communicable disease panel (Syphilis, Gonorrhea, Tuberculosis) labs be completed, along with verification that MMR, Varicella and Tdap be within CDC Guidelines.

I-693 form is not required you to bring in, we complete all paperwork in-house.

Once examinations, lab work and vaccinations are complete, you will be provided with a sealed envelope and a copy of your paperwork. The envelope should be sent to immigration with-in 60 days of the Civil Surgeon’s Signature date.

what is required to complete your paperwork?

  • Vaccine history (Vaccine history card could be in any language, we are able to transcribe)

  • Insurance Card

  • Passport, Employment card or Driver’s License


Fee

  • The Medical Physical and Paperwork is $330.00. PLEASE NOTE - AS OF MAY 1ST THE PRICE WILL BE INCREASED TO $350.00.

  • Lab work depends on the age and on the vaccinations and immunizations records that you provide me with. Health insurance can be used for this portion of the exam.

  • We no longer provide vaccinations/immunizations, you will need to obtain them from your primary care physician or the pharmacy. All local pharmacy’s, Walmart and Target will have the vaccinations. For those paying out of pocket, we advise to get a Good RX card (www.goodrx.com) and search for the cheapest price in your area.
