What does a physical Exam consist of?
The Physical Exam consist of the following
Pulse rate & rhythm, height and weight
Blood Pressure
Urinalysis - to provide readings of protein, blood and sugar
Vision - Driver must have an acuity of at least 20/40
Hearing
Physical Examination - exam consist of mental and physical wellness
IF You have cardiac/heart Disorder
There are various types of cardiac and heart disorders with different regulations. To speed up the process of your visit and required document, it is recommended to provide the last progress note from your Cardiologist.
if you have Monocular vision/visual problems
You must provide a copy of your most recent exam with your Eye Specialist.
If you have Hypertension
Remember to bring a list of your medications with you to your scheduled exam.
Sleep Apnea
If you have sleep apnea or a sleep disorder and use a breathing machine at night, you will must provide a recent usage report. Contact the phone number on your machine and have the usage report faxed to our secure fax at (508) 580-1116 prior to coming in for your appointment.
if you have diabetes
There has been several changes to the Federal regulations for Diabetes as of December 2019. Please review the information below for more details and requirements
For all Non-Insulin Diabetes you are required to provide your most recent HgA1c (Hemoglobin A1c) with in the past three months.
For all Insulin Dependent Diabetes, you are required to provide the Federal form ITDM Form MCSA-5870 https://www.fmcsa.dot.gov/sites/fmcsa.dot.gov/files/docs/regulations/medical/422521/itdm-assessment-form-final.pdf
The ITDM Form must be completed by your Endocrinologist prior to being seen by a medical examiner. Along with the ITDM form, you must provide glucose monitoring records from the past three months from your Endocrinologist and your most recent glucometer readings that have been downloaded from your glucometer.
